Registration Process


   The Student Registration and Enrollment Process Must be Completed in its Entirety

*First Day Exclusion Policy

It is a state of Illinois requirement that all children entering Preschool, Kindergarten and 6th grade submit a completed Certificate of Physical Health Examination Form (including documentation of all required immunizations).  If a child’s required physical and immunizations are not submitted prior to the first day of school attendance, delay in enrollment WILL occur. All physicals and immunizations forms must be documented on the Illinois approved the form and be reviewed and certified by the school nurse prior to the student being enrolled and attending school. Prior to submitting your child’s completed medical form(s), parents are advised to make a photocopy for their records, as copies will not be made at the schools or District Office. STEP 1 of the Registration Process: Complete online Registration

Computers are available at the school buildings by appointment only. (9am - 11am & 3pm - 3:30pm) You will be required to reschedule if you are 15 minutes late.

STEP 2 of the Registration Process: Provide the school main office with proof of identity and residency documentation. *Call for an appointment.

STEP 3 of the Registration Process*: Provide the school main office with a completed and signed copy of the Illinois Certificate of Physical Health Examination Form (*required for children entering Preschool for the first time, Kindergarten and 6th grades).

STEP 4 of the Registration Process: Parents/guardians will be notified if all required information has NOT been received and approved.
